What to Expect

Our team is committed to being there for patients throughout their cardiovascular care journey, serving as a trusted resource and partner in healing. We're here to provide support, answer questions, and help you get back to a life you love.

Preparing For Your Appointment

Once you have scheduled an appointment, please arrive 15-20 minutes beforehand to fill out the paperwork in our office.  The necessary forms for your visit include the registration form, patient consent form, electronic prescribing and demographic form, and your corresponding doctor’s history form.

What To Bring With You:

  • Insurance card(s)

  • Photo identification, such as a driver's license

  • Referral form (for HMO insurance, if required)

  • List of current medications you are taking including prescriptions, over-the-counter medications and any herbs (do not hesitate to just bring in the bottles if you are unsure)

  • Current pharmacy information

  • Relevant information about your medical and surgical history

Accepted Insurances

Our doctors participate in most major health plans, including HMO's. Some of the most common insurances are Medicare, Blue Cross Blue Shield, Blue Care Network, Health Plus, HAP and Cofinity. It is important that you check with your health plan to verify that they will cover visits, testing and procedures from our doctors. After your initial visit with us, we will contact your doctor to request referrals for up-coming appointments, but it is still your responsibility to notify your primary care doctor of you appointment here. If your plan has a co-pay, we ask that you pay it at the time you sign in for your visit. If you happen to incur a large balance, we will work with you on arranging a payment schedule that will fit your needs.

Scheduling

We understand conflicts may arise with your scheduled appointment time. Your follow up with us is very important to your health and, if you do need to reschedule an appointment, we ask that you please be courteous and give as much notice as possible. Our doctors always have a waiting list and early cancellations allow us to accommodate someone else. By the same token, we hope you understand that occasionally there will be times when we need to reschedule your appointment due to unforeseen changes in our physician schedules or emergencies. We will make every effort to find another convenient time for you as close to your original appointment as possible.
